How to creat simple google form

1. Login to Google Forms

2. Click on Contact Information template under Template gallery

3. Click on Customize Theme

4. Click on Image uploaded into theme options

 

5. Select Image for the header of form

6. Select theme colors and font style

7. Click on add question button to add question:

8. Select Checkboxes in type of question, type your question and add options:

9. Give a title to your google form and click on eye button to review the form:

10. Review your form:


 

11. Click on send button on main page of google form and fill the send form (you can also copy a link of the form as well to share it by other means). Send it to your personal email id, so you can fill the form later.

Click on Send

Fill the send form with email id, subject and message and click on Send

 

12. Click on Response tab (where you can see nothing as no one has responded to your form yet)

13. Fill the form you have received into your inbox.

 

14. Click on Responses tab again once you receive responses. Below you can view 2 responses as an example.

You will see a graph for your checkbox question in summary view.

 

15. You can also view your responses Question wise:

You can select question from drop down box to view the response

You can also expand options by clicking on view options for question 5

16. Create Google Sheet for your responses received:

Click on Crate Spreadsheet button

Verify name of the sheet and click on Create

You will be redirected to google sheet you have just created

17. Download CSV file of responses you have received through your google form:

Click on three dots and then click on Download responses (.csv)

Save the CSV file into your PC at your desired place.

18. Open CSV file in your PC to review the data:

Double click on CSV file that you have just saved on your PC, it will show a dialog, Click on OK.

Here it is your CSV file of the responses you have received through your google form.

You can use your CSV file for future reference or if you want to create any database in Google itself.
